Medical Social Services

Our medical social services offer a range of support services by our licensed medical social worker to our clients and families in their homes. We designed this service to help clients and their families cope with the emotional, social, and financial challenges of a chronic illness, injury, or disability. Some of the services offered by a medical social worker in home health care may include:

• Assessing the client’s and family’s needs and developing a care plan
• Assisting with access to community resources and financial assistance programs
• Providing counseling and support to clients and their families
• Advocating for the client’s rights and needs in the healthcare system
